On Your First Visit:

On your first visit, you will want to bring the following items:

1. Patient forms (download below)
2. Prescription for physical therapy from your referring physician
3. Insurance card and/or other medical coverage
4. Any co-pays or deductibles due (we accept cash, check, Visa, MC, or Discover Card)
5. Appropriate clothing and proper footwear (you most likely will be doing exercise)
