Jeff Kitchen, PT, Owner, Physical Therapist

Jeff has been a resident of the Valley since 1991. He is a 1982 graduate of the University of Iowa, where he earned his undergraduate degree as well as his Master’s degree in Physical Therapy. He moved to Arizona to further his studies, receiving his two year accreditation in orthopedic and sports manual therapy from the Norwegian Sorlandts Institute. In addition, he is a former Certified Athletic Trainer and a Certified Sports and Conditioning Specialist. From 1995-2000, Jeff supervised the athletic training program for the five high schools in the Scottsdale school district. Currently he is in private practice and contracts with Notre Dame Preparatory Academy to provide Athletic Training services for their program. He specializes in sports rehab, treating those with neck, back and extremity dysfunctions. Jeff is licensed with the state of Arizona as a board certified Physical Therapist.

Herman Fazz, Exercise Specialist

Herman is our Administrator of Post Rehab Programming and Exercise Science. Herman was raised in Yuma, AZ and graduated from Kofa High School. He graduated from Arizona State University in 2002 with a Bachelor’s degree in Exercise Science, Health and Wellness. Herman has been an employee at Rehab Plus Physical Therapy for over seven years. He is currently certified as a Performance Enhancement Specialist through the National Academy of Sports Medicine. Aside from working in the clinic, Herman enjoys training individuals, spending time with his wife Andrea and son Aedan, playing guitar and working out.

Jerod Torrey, MAT, ATC, LAT, Athletic Trainer

Jerod graduated high school in 1999 from Centerville, IA where he was a three time state champion in track and field. He continued his education at Iowa State University where he received his Bachelor’s degree in Sports Medicine and also competed as a sprinter for the Cyclones in the Big 12 Conference. Jerod was a two time NCAA Regional Qualifier as a Cyclone graduating in May of 2005. After college, Jerod moved to Payson, AZ and helped to start the first athletic training program for Payson High School and Payson Regional Hospital. After a two year stint in Payson, Jerod came to Rehab Plus Physical Therapy and Notre Dame Preparatory where he is currently working as the head Athletic Trainer for the Saints.
